Notice of Privacy
Effective July 1, 2017: Dockedin servers will be transmitting your health information to the online doctor selected by you for health care services, treatment plans, and payment. We will not use or disclose your health information for reasons other than when permitted or required by law, without your written authorization. You can rescind this authorization in writing at any stage, but we cannot withdraw or void any prior uses or disclosures of your health information already made with your authorization.
Right to an Accounting: You may request a list of the entities or persons (other than yourself) to whom Dockedin has disclosed your health information without your written authorization. This listing would not include any disclosures for health care services, treatment plans, payment, and certain other disclosures exempted by law. The written request for a listing of the disclosures should be signed and dated, and must identify the time-period of the disclosures and online doctors who provided the service about which you want the listing. We will not list any disclosures made prior to July 1, 2017, or those made earlier than 6 years before your request. Your request should specify the format in which you want the list (e.g. paper or electronic). You must submit your written request to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598. We will respond to you within 60 days. We will give you the first listing for any 3-month period free, but will charge you for all other accountings requested within the same 12 months.
Right to Amend: If you feel that health information we have about you is incomplete or incorrect, you can request us to amend your health records. Your written request for an amendment should be signed and dated, must identify the records you wish to amend and the online doctors who provided the service, and give the reason for your request. You must address your request to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598. The Dockedin Privacy Office will respond to you within 60 days. We may reject your request. If we do so, we will tell you why and explain the options.
Right to Inspect and Obtain Copy: You have the right to inspect and download a copy of your completed health records unless the online doctor who provided the service believes that disclosure of that information to you could be of harm to you. You may not inspect or obtain a copy of information gathered for legal proceedings or certain research records while the research is ongoing. Your request for a copy of the records must be submitted in writing, signed and dated, to the Dockedin Privacy Office. Requests for billing records should be sent to the billing departments. We may charge a fee for processing your request. If Dockedin denies your request to inspect or obtain a copy of the records, you may appeal the denial within Dockedin.
Right to Request Restrictions: You have the right to ask us to restrict the uses or disclosures we make of your health information for health care services, treatment plans and payment, but we do not have to agree. You may also ask us to restrict the health information that we use or disclose about you to someone who is involved in your care or the payment for your care, like a friend or family member. Again, we do not have to agree. A request for the limitation must be signed and dated, and you must identify the online doctor who provided the service. The request should also describe the information you want limited, say whether you want to restrict the use or disclosure of information or both, and tell us who should not be allowed to obtain this information. You must submit your written request to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598. We will inform you whether we agree or disagree with your request. If we agree, we will honor your request unless the information is required to provide you with emergency treatment.
Right to Request Communications Confidentially: You have the right to request in writing that we communicate with you about your health in a certain way or location. For example, you can ask that we contact you only by mail or at work. Your written request for confidential communications must be signed and dated. It should also identify the online doctor who provided the service, the confidential communications required, and specify how or where you wish to be contacted. You do not have to tell us the reason for your request. You must send your written request to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598. We will accommodate all reasonable requests.
Right to a Paper Copy of This Notice: You can view this Notice at www.Dockedin.com and may also ask us to send you a copy of this Notice. Even if you have arranged to receive this Notice electronically, you are still entitled to obtaining a paper copy of this Notice by submitting a written request to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598.
Complaints: If you believe your privacy rights have been violated, you may file a complaint with Dockedin or the Secretary of the US Department of Health and Human Services. To file a complaint with Dockedin, you must submit your complaint in writing to the Dockedin Privacy Office, 1327 Echo Hill Path, Yorktown Heights, NY 10598. You will not be penalized for filing a complaint.
Changes to This Notice: Dockedin may change this Notice at any stage. Any change in this Notice could also apply to health information we already have about you and any information we receive in the future. We will place a copy of the most current Notice on the Dockedin portal www.Dockedin.com. The effective date of the Notice will be on the first page.
